PARTICIPANT WAIVER, RELEASE OF LIABILITY & PHOTO CONSENT
By registering for and/or participating in this Charity Walk/Fundraiser for DADH, Inc PINK PUMPKIN BREAST CANCER AWARNESS PROGRAM, I acknowledge that participation in the Event involves physical activity and may involve certain risks, including, but not limited to, falls, injuries, illness, weather-related conditions, traffic, and other unforeseen circumstances.
I voluntarily assume all risks associated with my participation in the Event. To the fullest extent permitted by law, I hereby release and hold harmless DADH, Inc its officers, directors, employees, volunteers, sponsors, partners, agents, and representatives from claims or liabilities arising from my participation in the Event, except to the extent such release is prohibited by applicable law.
I certify that I am physically able to participate in the Event and understand that I am responsible for determining whether participation is appropriate for my health and physical condition. In the event of an emergency, I authorize Event personnel to seek reasonable emergency medical assistance on my behalf when necessary. I understand that I am responsible for any medical expenses incurred.
I understand that photographs, video, or other recordings may be taken during the Event. I grant DADH, Inc permission to use my image or likeness for promotional, educational, social media, website, and other organizational purposes without compensation, unless I notify the organization in writing that I do not consent.
If I am registering a participant under the age of 18, I certify that I am the participant’s parent or legal guardian and have authority to accept this waiver on their behalf.
