Liability Waiver and Release of Claims
I, the undersigned participant (or the parent/legal guardian if the participant is under 18), acknowledge and agree to the following in consideration of being permitted to participate in this table tennis event:
1. Assumption of Risk: I understand that participation in table tennis and related activities involves inherent risks, including but not limited to physical injury, illness, slips, trips and falls, collisions, muscle strains and sprains, contact with equipment or other participants, and in rare cases serious injury or death. I knowingly and voluntarily assume all such risks, whether known or unknown.
2. Release and Waiver of Liability: I hereby release, waive, discharge, and hold harmless the event organizers, sponsors, host venue (Manteca Senior Center) and its owners, officials, coaches, staff, and volunteers, together with their respective agents and representatives (the 'Released Parties'), from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury to me or my property that may occur while participating in, practicing for, or traveling to and from the event, whether caused by negligence of the Released Parties or otherwise.
3. Medical Treatment: I authorize the Released Parties to arrange or administer emergency first aid or medical treatment if it is deemed necessary, and I accept responsibility for any costs associated with such treatment.
4. Fitness to Participate: I confirm that the participant is physically fit and has no medical condition that would make participation unsafe, or that any such condition has been disclosed on this form.
5. Media Release: I grant the Released Parties permission to use photographs, video, or audio recordings taken during the event for promotional or informational purposes, without compensation.
6. Rules and Conduct: I agree to follow all event rules and the decisions of officials, and to conduct myself in a safe and sportsmanlike manner.
7. Parent/Guardian Consent (for minors): If the participant is under 18, I certify that I am the parent or legal guardian, that I have read and understood this waiver, and that I consent to the minor's participation and sign on their behalf.
