  • Lima Dance Academy recognizes our obligation to make sure our students and their parents are aware of risks and hazards involved in the sport of dance. By signing this waiver you release Lima Dance Academy and its employees from all claims on account of any injury which may be sustained by you or your child while attending any dance class, event or outside performance. You also affirm you now have and will continue to carry proper primary medical, health, hospitalization and accident insurance which you consider adequate for the protection of both your child and Lima Dance Academy. 

  • As a participating member of our 2026/2027 season you acknowledge your responsibility in paying monthly tuition, any associated costumes and all other communicated costs involved. This includes but is not limited to:

    1. The yearly registration fee of $35 (per family) to be paid at the time of registration.

    2. Recital costume fees due on November 16th and that failure to do so will result in the costume not being ordered. 

    If payment for tuition or costumes is paid after the due date, a late fee of $15 will automatically be assessed. There are no refunds on any payment remit to the studio. Credit may be issued at the discretion of Lima Dance Academy.
