Important Information

Mosaic Diagnostics respects your privacy and your choices about your genetic data. When you submit this form, you are asking us to delete genetic data that allows us to perform future DNA Signature testing.

Some records cannot be deleted. As a CLIA certified clinical laboratory, Mosaic Diagnostics is required by law to keep certain records for a set period of time. These may include your test order details, testing records, and previously issued PDF laboratory reports. We will keep these records only as required or allowed by applicable law and laboratory requirements.

Some records can be deleted. We will delete any raw genetic files that contain sequenced genetic data that may not have been used to produce the test result. Where we have used an outside CLIA certified laboratory, we will communicate your request to them as well.

After your raw, genetic, DNA Signature Data is deleted, it will no longer be available for future DNA Signature testing. If you want DNA Signature testing in the future, you will need to provide a new sample and have your DNA tested again. Additional fees may apply.
