Animal Surrender & Relinquish Form

1. Voluntary Surrender
I, the undersigned owner, hereby voluntarily surrender the above-listed animal to Pawsitively Posh Pet Rescue (“the Rescue”). I confirm that I am the legal owner and have the full authority to surrender this animal.

2. Transfer of Ownership
Upon signing this agreement:

• All ownership rights, interest, and claims to the animal are permanently transferred to the Rescue.
• 
  The Rescue will have full authority to determine all future care decisions, including veterinary treatment, placement, adoption, or humane euthanasia if medically required.

3. No Guarantee of Return
I understand that once the surrender is complete:

• I cannot reclaim the animal.
• The Rescue is not obligated to provide updates, visitation, or adoptive information.

4. Medical and Behavioral Disclosure
I confirm that:

• I have disclosed all medical history, behavioral concerns, medications, incidents, and veterinary records to the best of my knowledge.
• I understand that failure to disclose may put the animal, staff, or other animals at risk.

5. Financial Responsibility
I understand and agree that:

• All previous outstanding veterinary costs, licensing fees, or damages incurred before surrender remain my responsibility.
• Once surrendered, all costs moving forward are the responsibility of the Rescue.

• Optional donation toward care (not required but appreciated)

7. Representations
By signing below, I certify that:

• The animal has never bitten a human or another animal, OR
  has history of biting/aggression
• The information I have provided is accurate and truthful.
• The animal is not stolen, and no other person or entity has ownership rights.

8. Release of Liability
I release Pawsitively Posh Pet Rescue, its staff, volunteers, and affiliates from any liability arising from the surrender of this animal. I will not dispute or challenge ownership now or in the future.
