    Help us recognize the individuals and organizations making a meaningful impact within ISFA and the countertop and surfacing industry. Use this form to submit nominations for the four Member Award categories listed below. Nominators may submit one nomination per category. Select the appropriate category and provide the requested information about your nominee, including specific examples of their accomplishments, contributions, leadership, service, or industry impact. Please provide thoughtful and detailed responses to help the judges understand why your nominee is deserving of recognition.     Consent and Use of Likeness

    I hereby grant the International Surface Fabricators Association (ISFA) permission to use my likeness in photographs, video, or other digital media (collectively, “Photos”) in any and all of its publications, including print materials, web-based publications, marketing content, and promotional materials, without payment or any other consideration.

    Use of Material and Fabrication Shop Images

    This release also extends to photographs or video that depict project materials, completed work, equipment, and fabrication facilities. I confirm that I have the authority to grant ISFA rights to use such images and that no proprietary or confidential information is violated by their publication.

    Ownership and Rights

    I understand and agree that all Photos will become the property of ISFA and will not be returned. ISFA reserves the right to edit, alter, copy, exhibit, publish, or distribute these Photos for any lawful purpose. I waive the right to inspect or approve the finished product wherein my likeness or submitted materials appear and waive any right to royalties or other compensation arising from or related to the use of the Photos.

    Use Across Channels

    This release includes, but is not limited to, the use of Photos in:

    ISFA websites and web-based publications
    Print and digital advertisements
    Social media and digital campaigns
    Newsletters and organizational bulletins
    Award submissions and related promotional material
    Third-Party Photographer Permissions

    If the submitted photos were taken by a professional photographer or any third party who may retain copyright, I understand that I am responsible for obtaining and providing written permission or a release from the photographer, granting ISFA the right to use the photos as described in this policy.

    Release and Waiver

    I hereby hold harmless, release, and forever discharge ISFA from all claims, demands, and causes of action which I, my heirs, representatives, executors, administrators, or any other persons acting on my behalf or on behalf of my estate may have by reason of this authorization.
