  • * I acknowledge and agree to the University of Wyoming's cancellation policies: over 30 days refund 75% of cost, between 15-29 days refund 50% of cost, and 14 days or less no refunds

  • * I acknowledge that my registration is NOT complete until I pay the course tuition via UW's website. Payment link will be provided upon submission of this registration form.

  • * I acknowledge and agree to LWM's course policies including course cancellation and tuition refunds (see above for UW policies).

  • * I have read the WFR Welcome Letter and acknowledge that the Hybrid WFR course requires the 100% completion of 35 hours of course work prior to the start of the in-person component.

  • * I have reviewed the course completion timeline in the WFR Welcome Letter and acknowledge that completion of the online course materials requires submission and instructor approval of written assignments prior to the start of the in-person course.

  • * I acknowledge that if I do not complete the online course materials prior to the start of the in-person component that I will not be able to continue on with the course and that a tuition refund will not be given.
