1. I understand that nail services, facials, body waxing, brow waxing, and brow tinting carry potential risks, including but not limited to allergic reactions, irritation, redness, sensitivity, burns, skin lifting, cuts, nail damage, or infection.
2. I release and hold harmless Polished by Kaci and the technician (Kaci) from any liability for injury, loss, or damages that may result from the services provided.
3. I confirm that I am voluntarily receiving services and that they are being performed in a private home environment.
4. I agree not to pursue legal action against Polished by Kaci or Kaci for any reason related to services received.
5. I confirm I have disclosed any known allergies, medical conditions, medications, skincare products, or recent skin treatments that may affect the service.
6. I understand that services are being provided by an independent nail technician (Kaci) and agree not to hold the homeowner or property owner liable for any incident, injury or claim arising from services performed.
I am the parent/legal guardian of the above minor. I give permission for them to receive nail services, facials, body waxing, brow waxing, and/or brow tinting, and I acknowledge and accept the risks and terms listed in this form.
This waiver and release applies to all services described in this form provided by Polished by Kaci from the date signed onward, unless revoked in writing. Revocation applies only to services provided after written notice is received.
