By acknowledging below, you certify that the information provided is accurate to the best of your knowledge. You authorize RoddsWorldd Entertainment to use aggregated and de-identified information collected through this form for program evaluation, grant reporting, strategic planning, and impact measurement.

LIABILITY WAIVER

I, {ParticipantName}, understand that my participation involves physical activity, dance, movement, and related activities that may involve inherent risks, including the risk of injury. I,{ParticipantName}, voluntarily choose to participate and acknowledge that I do so at my own risk.

To the fullest extent permitted by law, I agree to release, waive, and hold harmless RoddsWorldd Entertainment, its employees, instructors, contractors, volunteers, representatives, affiliates, and partners from any and all claims, demands, damages, liabilities, or causes of action arising out of or related to my child’s participation, including claims for personal injury, illness, property damage, or loss, except to the extent such liability cannot legally be waived or released.

MEDIA WAIVER

I, {Name}, authorize RoddsWorldd Entertainment (“RWE”) and its employees, instructors, contractors, volunteers, representatives, affiliates, and program and venue partners to photograph, film, video record, audio record, and otherwise document my child’s participation in Oldway Sessions.

I grant RWE and its authorized partners the irrevocable, worldwide, royalty-free right and permission to use, reproduce, edit, publish, display, distribute, and otherwise use these photographs, recordings, and other media featuring my child for educational, promotional, marketing, archival, fundraising, grant reporting, program evaluation, and other organizational purposes, in any format or media now known or later developed, without compensation or additional approval.

I understand that these materials may be used in print and digital materials, websites, social media, publications, presentations, grant and funding materials, and other communications related to RWE and its programs. I waive any right to inspect or approve the final materials and release RWE and its authorized partners from claims arising from the permitted use of these materials, except where prohibited by law.
