If you are requesting Accommodations for students, be sure to complete the IEP/504 Accommodation Request Form (link available on the 'Auditions' page) and upload to this submission along with supporting documentation. Remember that in order for these accommodations to be approved, they must be using the same accommodations in your classroom.
