Zine Collection Submission Form

Please review the following requirements before submitting this form:

1. Please fill out this form only if you are the zinemaker/author of the zine you are submitting.
2. Please fill out a form for each individual submitted zine.
3. Please review the submission guidelines on our Zine Collection page.
4. Please submit the physical copy at the Central Library, Adult Reference desk, no later than a week after this form is submitted.
