This year we are again proud to be in partnership with Operation Warm to help provide low cost high quality coats to our local children.

Due to limited availability and sizes filling out this application does NOT GUARANTEE that your child/children will receive a winter coat.

If your child/children are selected to receive a coat it will be FREE OF CHARGE. Your child/children must be present to receive the coat to ensure that the coat fits them properly.

By signing this application you as the parent/guardian are giving CAM Outreach Ministries permission to take your child/children's photo wearing the coat. These photos will be used on our website, social media and in marketing materials by our organization.
