• Document Signing & Notarial Services – Intake Form

Please complete this intake form so Law Notary LLC can review your notarial service needs and help ensure you are properly prepared for your appointment.
  • Providing accurate information about your documents, signers, identification, witnesses, and appointment needs will help us review your service request and identify anything that may require attention before your appointment.

  • IMPORTANT: Please do not sign, date, or initial your document(s) before your appointment unless specifically instructed to do so by the document issuer, receiving agency, or another authorized party.

  • PLEASE NOTE: Law Notary LLC is not a law firm and does not provide legal advice, select or prepare legal documents, determine the legal effect of documents, or advise you how a document should be completed.

  • CONTACT INFORMATION

Please provide the contact information for the person coordinating this notarial service. This person does not have to be the signer.
  • Format: (000) 000-0000.
  • How would you prefer that we contact you regarding this request?*
  • Are you one of the individuals who will be signing the document(s)?*
  • If you are coordinating this service for someone else, what is your relationship to the person(s) needing the service?*
  • DOCUMENT & NOTARIAL SERVICE INFORMATION

Please tell us about the document(s) included in your service request. If you are unsure whether notarization is required or what information to provide, that is okay. Please provide the information you have so Law Notary LLC can review your service request and appointment needs.
  • What type(s) of document(s) are included in this service request?*
  • Example: Power of Attorney; Affidavit of Residency; Passport Copy Certification

  • Does the document already contain notarial wording or a notarial certificate for the notary to complete?*
  • NOT SURE? Notarial wording is typically located near the signature area or at the end of the document and may include wording such as “Acknowledged before me,” “Subscribed and sworn,” “Verification on Oath or Affirmation,” or similar language, along with an area for the notary's signature and seal.

If you are unsure whether your document contains notarial wording, Law Notary LLC can review the existing document and explain the notarial wording that appears on it.

  • NO NOTARIAL WORDING: If your document does not contain notarial wording or a notarial certificate, a loose notarial certificate may need to be attached. A notary cannot select the notarial act or certificate on your behalf. Please contact the document preparer, issuing agency, receiving party, or an attorney to determine the type of notarization required. Once you have been instructed which notarial act is required, Law Notary LLC can provide the appropriate loose certificate for completion at the appointment.

  • DOCUMENT REVIEW AVAILABLE: If you are unsure whether notarial wording is included, please email the document or applicable signature/notarial page to lawnotary47@gmail.com for review. We can identify and explain notarial wording already contained in the document and let you know whether additional information is needed before the appointment.

  • Other than signatures, dates, and initials that are intended to be completed during the appointment, are all other applicable sections of the document completed?*
  • IMPORTANT: Incomplete documents may prevent the notarization from proceeding. Please do not sign, date, or initial your document(s) before the appointment unless specifically instructed to do so by the document issuer, receiving agency, or another authorized party.

  • Did an attorney, agency, employer, financial institution, title company, government office, document recipient, or other party provide you with instructions or requirements for signing, witnessing, or notarizing the document(s)?*
  • DOCUMENT REVIEW: If you have documents, written instructions, or correspondence that you would like Law Notary LLC to review before your appointment, please email them to lawnotary47@gmail.com. Please include your full name in the subject line of the email so we can match the documents to your intake form.

  • COPY CERTIFICATION INFORMATION

Please provide the following information so we can review your copy certification request and prepare for your appointment.
  • Which documents in this request need copy certification?*
  • Examples: Passport, driver's license, diploma, identification document, or other record.

  • Please enter the total number of documents for which copy certification is requested.

  • Will you have the original document available at the appointment?*
  • How will the copy or copies be provided for the appointment?*
  • CUSTOMER-PROVIDED COPIES: Please bring the original document and the copy/copies to your appointment. Each copy must be printed on a full-size sheet with sufficient blank space for the copy certification notarial certificate. When possible, please leave approximately the bottom one-third (⅓) of the page blank for the notarial certificate, signature, and seal.

  • COPY PRINTING: If you need Law Notary LLC to print your copy or copies for the appointment, please email the document files(s) at least 3 hours before your scheduled appointment to lawnotary47@gmail.com. Please include your full name and appointment date/time in the email so the documents can be matched to your appointment. Law Notary LLC will format the printed copy with appropriate space for the copy certification notarial certificate.

  • COPY CERTIFICATION REVIEW: Not every document is eligible for notarial copy certification. Law Notary LLC will review the document type before confirming the service. If you are unsure whether your document can be copy certified, please provide the document details through this intake form before scheduling.

  • SIGNER INFORMATION

Please provide information about each person who will be physically present and signing during the appointment.
  • Will all individuals who need to sign be present for the appointment?*
  • Will any person who will be physically present and signing be signing on behalf of another person, business, trust, estate, or other entity?*
  • Does any person who will be physically present and signing during the appointment have a physical limitation or other circumstance that may affect how they are able to sign?*
  • Can each person who will be physically present and signing communicate directly with the notary during the appointment?*
  • IDENTIFICATION

Each person whose identity must be established for a notarial act must be prepared to provide satisfactory evidence of identity in accordance with applicable Indiana notary requirements. Please answer the following questions so potential identification issues can be reviewed before the appointment.
  • Does each signer have a current, unexpired government-issued photo ID available to present at the appointment?*
  • What type of government-issued photo identification does each signer expect to present at the appointment?*
  • Does the name shown on each signer's identification reasonably correspond with the name shown for that signer on the document(s)?*
  • Is there any known issue that may prevent a signer from presenting acceptable identification at the appointment?*
  • IDENTIFICATION NOTICE: Information provided on this intake form is used for appointment screening and preparation only. Final determination of whether identification presented is satisfactory for the requested notarial act is made by the notary after examining the identification and circumstances at the appointment.

  • If any signer may not have acceptable photo identification, is another method of identification expected to be used?*
  • CREDIBLE IDENTIFYING WITNESS: A credible identifying witness is a person who may be used to help establish a signer's identity when the signer does not have acceptable identification. This is different from a witness who signs the document. Specific requirements apply, and Law Notary LLC must review the circumstances before confirming whether a credible identifying witness may be used.

  • Do you already have a person available to serve as the credible identifying witness?
  • NO CREDIBLE IDENTIFYING WITNESS AVAILABLE: If you do not currently have a person available to serve as a proposed credible identifying witness, or you are unsure, please contact Law Notary LLC before scheduling or attending the appointment so the identification circumstances can be reviewed. A credible identifying witness is not simply someone who has identification; specific requirements must be satisfied.                                                                                        Law Notary LLC does not automatically provide a credible identifying witness.

  • Does the proposed credible identifying witness personally know the signer?*
  • CREDIBLE WITNESS REVIEW NEEDED: A credible identifying witness must personally know the signer. If the proposed witness does not personally know the signer, or you are unsure whether the requirements can be met, Law Notary LLC will need to review the identification circumstances before the appointment. Please do not assume that bringing another person will automatically satisfy the identification requirement.

  • How will the proposed credible identifying witness establish their identity to the notary?*
  • WITNESS REQUIREMENTS

Some documents require one or more witnesses as part of the signing or execution process. Please review your document and any instructions you received to determine whether witnesses are required.
  • Do your document(s) or instructions indicate that one or more witnesses are required?*
  • NOT SURE ABOUT WITNESSES? Please review the signature page(s) and any instructions provided with your document to determine whether witnesses are required. If you are still unsure, you may email the applicable document or signature page(s) to lawnotary47@gmail.com for review before your appointment.

Law Notary LLC can identify witness signature areas or instructions that appear in the document but cannot determine or advise whether witnesses are legally required.

  • How will the required witness(es) be provided?
  • CUSTOMER-PROVIDED WITNESSES: If you are providing your own witness(es), please ensure they are available for the entire signing and prepared to present identification if required. Additional witness qualifications may depend upon the document or instructions you received.

  • WITNESS COORDINATION: If you request assistance coordinating a witness, Law Notary LLC may use its Signature Witness Program through the Law Notary Support Network to identify an available participant.

Witness coordination is $40 per witness.

Witness availability is not guaranteed and depends upon the appointment date, time, location, and any requirements applicable to the document or transaction.

  • Were you given any specific instructions or qualifications for the witness(es)?*
  • IMPORTANT: Law Notary LLC does not determine whether your document legally requires witnesses or select legal witness requirements for you. If the document or instructions do not clearly state the witness requirements, you may need to contact the document preparer, issuing agency, receiving party, or an attorney for guidance before the appointment.

  • Format: (000) 000-0000.
  • Has the signer been informed that a notary will be visiting and that documents are expected to be signed during the appointment?
  • Before the appointment is confirmed, please ensure the signer is aware that a notary will be visiting and that documents are expected to be signed. The notary must be able to communicate directly with the person signing and confirm their willingness to proceed at the time of the appointment.
