    Please note:

    • Submission of this form does not guarantee placement.
    • Opportunities are offered throughout the year and are subject to availability and the requirements of the host organization.
    • For structured, year-round internships, the typical placement duration is approximately 6 weeks. If required hours or days are missed, the placement may be extended accordingly. Students are encouraged to complete their placement within a maximum of 8 weeks, subject to the requirements of the host organization.

  • By submitting this form, I confirm and acknowledge the following:

    1. I certify that all information provided in this application is true, complete, and accurate.

    2. I understand that submission of this form does not guarantee placement, and opportunities will be offered based on suitability, availability, and organizational requirements.

    3. I agree to be contacted by Eye On Ivy regarding current and future placements.

    4. If I am under 18 years of age, I confirm that I have obtained parental/guardian consent to submit this application. I have discussed the Student Placement Program with my parent/guardian and I am aware of all applicable program requirements.

    5. I understand that Eye On Ivy serves as a placement and coordination platform. The nature of work, tasks, schedules, policies, and responsibilities assigned during the placement are solely determined by the host organization. Any placement-specific matters should be discussed directly with the host organization while keeping the Eye On Ivy Placement Team appropriately informed. Eye On Ivy shall not be held responsible for the day-to-day operations or decisions of the host organization.

    6. I understand that the issuance of certificates is solely at the discretion of the host organization and is generally subject to the successful completion of the required placement hours, satisfactory performance, timely submission of assigned work, professional conduct, punctuality, attendance, and compliance with the organization's policies. Eye On Ivy will ensure that certificate is received on meeting all requirements.

    7. I understand that failure to maintain professional conduct, attendance, punctuality, or to complete assigned responsibilities may result in termination of my placement by the host organization without entitlement to a certificate.

    8. I understand that Eye On Ivy will not provide transportation to the placement site and that it is my responsibility to coordinate transportation.

    9. I am aware that absence without prior notification shall be considered a breach and may result in termination of the placement. The student must inform the Focal Person at the organization at least one day before the intended leave.

    10. I am aware that in case of a breach or withdrawal after signing up for a program, Eye On Ivy shall not consider the student for any future placement opportunities, and any fee submitted will be forfeited.

    11. I am aware that Eye On Ivy will place me with an appropriate organization aligned with my current area of study and future study major. I will not hold Eye On Ivy responsible for placing me with another organization if I fail to fulfill my commitments to the first referred organization.

    12. I am aware that I will be matched with a temporary and generally unpaid placement opportunity with an organization unaffiliated with Eye On Ivy or the school. The placement opportunity is educational and experiential in nature and is not offered as employment or as a substitute for paid work. Neither party shall be under any obligation to offer the student a continued position at the organization.

    By providing the information required in this form, I acknowledge my participation in the Student Placement Program and hereby release, discharge, and hold harmless Eye On Ivy, its employees, directors, representatives, and affiliated organizations from any and all claims, suits, actions, causes of action, damages, demands, or liabilities, including known or unknown claims, now existing or hereafter arising, in law, equity, or otherwise, that might directly or indirectly result from participation in this program.
