Important Deadline Information:
- Submission Window: This form and your inductee list must be submitted at least three weeks prior to your ceremony if you would like your certificates in time for your ceremony.
- Late Submissions: If submitted with less than three weeks' notice, we will send a temporary certificate template for use during your ceremony while official certificates are processed and shipped.
- Next Steps: Once you submit this form, keep an eye on your email for a personalized spreadsheet to enter your inductee details.
