Bus Rider Permission

I give permission for the member listed above to use the bus service provided by C4K for transportation from the following school to the C4K Clubhouse (945 2nd Street SE., Charlottesville, VA 22902) for the Fall 2026 program season:

NOTE:  We will be providing two pickups per week at Charlottesville Middle School (CMS) this season due to the move of 6th grade from Walker to CMS. Based on current registrations, we ask that CMS members select the Tuesday pickup if either day works to minimize crowding in the Clubhouse.

I understand that:

• The bus provides transportation to C4K only. It is the responsibility of the member’s guardian to arrange pick up from C4K by 7:30 PM.
• The bus will be running immediately after school and it is the responsibility of the member to arrive promptly for pickup.
• Members are expected to adhere to the same rules of conduct while on the bus as if they were in the Clubhouse.
• If the member is not at school, C4K should be notified that they will not be using the bus (phone / text: 434-270-0072, email: c4kmaker@c4kclubhouse.org)
• Continued access to the service will be at the discretion of C4K staff.  Taking a seat on the bus is a previlege that reduces availability for others.  Inconsistent ridership or disruptive behavior while riding will result in the loss of this previlege.
• There may be cancellations of the service or individual pick-ups /drop-offs due to availability, capacity, etc.  C4K will make every effort to notify of the situations well in advance.
