Includes the complete two-year Confirmation preparation program,
Confirmation retreat, and program supplies. Payment is due at the time of registration.

Welcome to the Faith Formation Program at St. Anthony of Padua Catholic Church. We are honored to accompany your family as your child prepares to receive the Sacrament of Confirmation. Our program helps young people grow in their relationship with Jesus Christ, deepen their Catholic faith, and become active members of the Church. Confirmation is much more than completing classes. It is the beginning of a lifelong commitment to Christ and His Church.

Parents and guardians are the primary teachers of their children's faith. Your support through Sunday Mass attendance, faithful class participation, required parent formation, and encouragement is essential to your child's faith journey.

REQUIRED PARENT FORMATION
Year One parents required "Life in the Spirit Seminar" on Wednesdays.
Year Two parents: required Parent Education Classes on Sundays.

CALLED TO PROTECT
Parents/guardians will receive Called to Protect information on preventing sexual, physical, and emotional abuse. A parent class will be offered at a later. Students receive age-appropriate safe-environment lessons, and parents will be notified in advance.

Communication is by email and text message. Parents must check both regularly, keep contact information current, and contact the Faith Formation Office during office hours.

I hereby release St. Anthony's Catholic Church in Forest Grove, the Archdiocese of Portland, and their employees, chaperones, and agents from liability and claims of any nature that may arise or result from participation in St. Anthony activities. I further authorize St. Anthony's Church and its employees, chaperones, or agents to secure necessary medical services for my child(ren) in the event of an accident or illness. I understand that I will be solely responsible for payment of those services.
