This form is for requesting a priest to visit a person who is unable to come to church due to their physical condition or circumstances.
Please use this form for someone who is:
- In hospital or receiving medical care
- Elderly or has limited mobility
- Living in an aged care facility
- Bedridden or unable to leave their home due to illness
- Recovering from an accident or serious health condition
- Requesting the Sacrament of Anointing of the Sick or Last Rites
Please note: A priest visit is intended for people who are genuinely unable to attend church. If the person is able to attend church but prefers to receive a visit at home, please do not use this form. We encourage those who are able to attend Mass to continue participating in the sacramental life of the Church at the parish.
