I acknowledge that I am participating in the BIG SPLASH WATERSLIDE YOUTH TRIP on AUGUST 27th 2026. As the event is taking place in a body of water and requires a ferry ride over a body of water to get there, I understand that participating in the Activity entails inherent risks of physical injury, including, but not limited to drowning. I acknowledge that in order to participate in this event I must have the ability to swim and release Saanichton Bible Fellowship & Youth For Christ South Island, the pastors, youth workers, the board of Elders, directors, officers, agents, officials, servants, organizers, representatives, the Ministry Staff, and any employees from any injuries sustained by me due to my swimming limitations or abilities.
SBF Church Permissions
Photo and Media Release for SBF Church
I hereby grant SBF Church and its legal representatives, employees, and volunteers the irrevocable right to capture, edit, alter, copy, exhibit, publish, or distribute photographs, video footage, and audio recordings (collectively, "Media") of my child while at SBF Church programs. This Media may be used for any lawful purpose related to the non-profit's mission, including but not limited to: organization websites and digital newsletter, official social media platforms (e.g., Facebook, Instagram), Printed brochures, posters, and fundraising flyers, Grant applications and community presentations, I acknowledge that all Media captured belongs solely to SBF Church and neither I nor my child hold any intellectual property rights over it. I understand that this authorization comes without any expectation or right to monetary compensation, royalties, or other benefits. I waive the right to inspect or approve any finished product or marketing copy where my child's likeness appears.
Travel and Transportation Permission (to, from, and for SBF Church Youth programs)

I grant permission for my child to travel in approved vehicles to, from, and for the purpose of SBF Church youth programs with approved SBF Church Staff or Volunteers.
Medical Release and Permission for Medical Care - SBF Church

I authorize SBF Church Staff and Volunteers to provide access to medical care for my child in the event of injury during any and all youth program participation.
Release of Liability and Hold Harmless agreement - SBF Church

By checking the box (left) I/we, the Parent(s) or Guardian named above, undertake and agree to indemnify and hold harmless the Pastors, the board of Elders, directors, officers, agents, officials, servants, organizers, representatives, the Ministry Staff, and any employees of Saanichton Bible Fellowship from and against any loss, damage or injury suffered by me/us as a result of being part of the activities of Saanichton Bible Fellowship, as well as of any medical treatment authorized by the supervising individuals representing the church. IF THE PARTICIPANT is under nineteen (19) years of age, in consideration of the Participant being permitted to engage and take part in programs offered, carried on, sanctioned or sponsored by Saanichton Bible Fellowship, the undersigned Parent or Guardian of the Participant hereby releases and undertakes and agrees to save harmless and keep indemnified Saanichton Bible Fellowship, the pastors, the board of Elders, directors, officers, agents, officials, servants, organizers, representatives, the Ministry Staff, and any employees from and against all claims, actions, costs and expenses and demands whatsoever in respect of death, injury, loss or damage to the person or property of the Participant, howsoever caused, regardless of whether same may have been contributed to or occasioned by the negligence of Saanichton Bible Fellowship, the pastors, the board of Elders, directors, officers, agents, officials, servants, organizers and representatives, the Ministry Staff, and any employees. It is hereby acknowledged that the contents hereof are fully understood by the Participant who agree(s) that same shall be binding upon (his/her/their) heirs, successors, executors, administrators and assigns.
