Permission Form and Medical
We, Parent/Guardian (full name)* (parent/guardian initial, over 18+) hereby give permission for our son/daughter; to travel from Calgary, AB to Maple Ridge, BC from Sept 12-13, 2026.

Our son/daughter is attending the Fort Regatta with the Calgary Canoe Club. Coaches and/or chaperones throughout the trip will supervise athletes.

I give my permission for medical assistance to be administered should they become ill or involved in an accident initial* (parent/guardian initial, over 18+).
CCC has collected information when you join the program but did not collect Health Card Numbers at that time. Please provide the athlete health card number before the trip (just in case we need a trip to the hospital).
Do you have out-of-province traveller medical insurance? Alberta health coverage has limitations when outside the province. You can find more on this at: https://www.alberta.ca/ahcip-what-is-covered.aspx
LAST THING: All Maple Ridge Regatta athletes must register with BC's ramp to sign waivers.
