Kinderdance of South Charlotte Enrollment

PARTICIPATION WAIVER AND RELEASE OF LIABILITY I, the undersigned parent or legal guardian, hereby give permission for my child to participate in Kinderdance, Kindergym, Kids Yoga, and/or related enrichment activities offered by Kinderdance South Charlotte. I understand that participation in dance, movement, fitness, and physical activity programs involves certain inherent risks, including but not limited to falls, collisions, strains, sprains, and other injuries. I voluntarily assume all risks associated with my child's participation. I certify that my child is physically able to participate in these activities and that I have disclosed any medical conditions, allergies, or special needs that may affect participation. In consideration of my child's participation, I release, waive, discharge, and hold harmless Kinderdance South Charlotte, its owners, instructors, employees, contractors, host schools, and affiliates from any and all claims, liabilities, damages, costs, or expenses arising out of or related to participation in the program, except where prohibited by law. I authorize Kinderdance South Charlotte to obtain emergency medical treatment for my child if I cannot be reached immediately. I understand that I am responsible for any resulting medical expenses. Please note: Class cancellation must be submitted at least 15 days before the next payment date. I acknowledge that I have read and understand this waiver and agree to its terms.
