Faces by Aysia: Makeup Appointment Form

I’m a MAC trained Makeup Artist and Makeup Enthusiast, servicing primarily Maryland and Virginia. I offer soft glams, traditional glams, and editorial makeup. I use high quality, effective products to deliver high quality glams for a competitive price. As a makeup enthusiast, I’ve done projects testing products and giving feedback for multiple cosmetic companies including Lancôme and Urban Decay. I have some travel availability; I can travel to a home, hotel, or venue. Let’s glam!

All makeup appointments require a $20 deposit that must be paid at least three days before to secure the appointment spot. The deposit will be deducted from the full appoitment cost. Appointments without a deposit will not be honored. Appointments usually take 1 hr to 1 and 30 minutes, please plan accordingly.

 I will reach out via text to confirm receipt of the deposit.

Policies

Please read this section in full before submitting your deposit.

 

Late and No Show Policies

1. Clients that are over 20 minutes late forfeit their appointment spot and their deposit

2. No show clients forfeit their deposit.

3. Clients who cancel their appointments within 48 hours of their appointment time forteit their deposit.

 

Travel Policies

The following policies apply for appointments where Faces by Aysia is traveling to the client.

1. Pets and small children must be closely watched during the appointment time for their safety. When working, I will be moving around the makeup chair and would hate to accidently step on any little feet!

2. The meeting location must have accessible parking. If parking is not readily available, please let me know in the notes section of this form.

3. Please expect me to arrive within 10 minute before your appointment time so I can set up my equipment.

 

On site Policies

The following policies apply for appointments where the client is travelling to Faces by Aysia.

1. Clients can have no more than two guests within my salon suite. I cannot guarantee additional seating for guests.

2. If a client is early for an appointment, I cannot guarantee to start the appointment before the scheduled time.

 

By submitting this form, the client and/or submitter of this form agree to abide by the aforementioned policies and understand that breaking any of these policies can result in the cancellation of the appointment at any time before or during the appointment. The client and/or submitter of this form also agree that Faces by Aysia is not liable for any internal or external reactions to products used during the appointment as a result of undisclosed allergies or sensitivities by the client and/or submitter of this form.
