WORKSHOP WAIVER, RELEASE & MEDIA AGREEMENT
1. General Liability Release
By signing below, I acknowledge that participation in this workshop and related activities involves inherent risks, including the risk of injury, illness, accident, property damage, or other loss.
To the fullest extent permitted by law, I voluntarily assume the risks associated with participation and agree to release and hold harmless:
- Super Shy Entertainment LLC
- Its owners, officers, employees, staff, contractors, instructors, choreographers, photographers, videographers, assistants, volunteers, agents, representatives, sponsors, and venue partners
- All other individuals and entities assisting with, participating in, or connected to the workshop
from claims arising out of or related to participation in the workshop, including claims involving injury, illness, accident, property damage, or negligence, except to the extent such a release is prohibited by applicable law.
This release applies to activities occurring before, during, and after the workshop that are related to the event.
2. Participant / Group Release
By signing below, I confirm that:
- I have read and understand this agreement.
- I agree to these terms for myself.
- If I am the parent or legal guardian of a minor participant, I consent to the minor’s participation and agree to these terms on the minor’s behalf to the extent permitted by law.
- If I am signing for additional participants, I represent that I have the legal authority to do so.
- Each participant covered by this agreement is subject to the applicable liability, medical, media, and other terms contained in this agreement.
Participants who are 18 years of age or older should sign for themselves unless another person has legal authority to sign on their behalf.
3. Medical Release
In the event of an injury, illness, or medical emergency, I authorize Super Shy Entertainment LLC and its authorized staff or representatives to obtain or arrange reasonable emergency medical assistance for me and, where I have legal authority, for any minor participant under my care.
I understand that I am responsible for any medical expenses incurred on behalf of myself or any participant for whom I am legally responsible.
4. Media Release
I understand that photographs, video recordings, audio recordings, interviews, and behind-the-scenes content may be captured during the Super Shy Entertainment LLC workshop.
By signing below, I grant Super Shy Entertainment LLC and its authorized representatives permission to photograph, film, record, and use my image, voice, likeness, statements, and performance, and, where I have legal authority, those of any minor participant for whom I am signing.
These materials may be used in connection with Super Shy Entertainment LLC and its programs, events, marketing, publicity, educational activities, and promotional efforts, including:
- Social media
- Websites
- Advertising and promotional materials
- Press and magazine features
- Television and streaming platforms
- Podcasts
- Documentaries
- Billboards
- Commercials
- Educational materials
- Trailers and highlight reels
- Future Super Shy Entertainment LLC programs and projects
This permission is worldwide, perpetual, royalty-free, and irrevocable, to the extent permitted by applicable law.
I understand that I will not receive compensation or royalties for the authorized use of these materials. I waive any right to inspect or approve the finished materials or the manner in which they are used.
The photographs, video, and other recordings will not be sold solely as standalone stock footage.
Participant Acknowledgment
By signing below, I acknowledge that I have carefully read and understand this agreement and voluntarily agree to its terms.
