LIABILITY POLICY I acknowledge the inherent risks involved in swimming and aquatic activities and voluntarily assume full responsibility for any injury, damage, or loss that may result from my child(ren)'s participation in this clinic. I hereby waive and release MARSTA LLC, its owners, and its staff from any and all liability, past, present, and future, relating to the services provided. By checking the box below, I confirm that I have read and fully understood this waiver.
PHOTO & VIDEO CONSENT I am the parent or legal guardian of the minor(s) named in this form. By providing consent below, I authorize MARSTA LLC to photograph and/or video record my child during clinic activities. I understand that these images and recordings may be used for marketing, promotional, educational, and informational purposes, including on websites, social media platforms, and printed materials. I acknowledge that no financial compensation will be provided. I understand that I may revoke this consent at any time by submitting a written request to leah@marstacoachingconsultant.com. Please note that images already published prior to revocation may not be retractable.
REFUND POLICY Full refunds are only provided 7 days or more prior to the start of the leadership workshop. No refunds will be issued within 6-days of the start of the first session, or once the workshop has begun. I have read and agree to the camp’s refund policy.
