TRANSPORTATION CONSENT & RELEASE FORM
I, blanks , the parent/guardian blank hereby give my minor child permission to ride in the vehicle of and/ore be driven by an active Student Volunteer screened and designated by the Lakewood Baptist Church Youth Ministry. I have discussed these arrangements with my child.
On behalf of myself and my child, I agree to release and discharge, and agree to indemnify and hold harmless, Lakewood Baptist Church and its leadership, employees, Student Volunteers, and/or any other persons or entities acting on its behalf, from any and all claims, rights of action and causes of action that may have arisen in the past or may arise in the future, directly, or indirectly, from the transportation of my child with respect to the above event.

I have read and agree to the Consent and Release provisions above.
