As a member of a mission team sponsored in whole or in part by Choirfire Missions, I agree to:

• Go as a servant-disciple of Jesus Christ and adopt that attitude when dealing with my fellow team members.
• Remember that I am an unknown Christian from another part of the world and will be watched very closely. I will not take this responsibility of setting a good example lightly.
• Not share my personal contact information (including phone number, address, or social media handles) with anyone during the trip. This is to protect both myself and my fellow team members.
• Accept the leadership authority of the team leader and abide by his/her decisions as they concern the mission trip.
• Attend all team meetings possible, prior to departure, during, and after the mission trip.
• Refrain from gossiping, complaining, and obscene or insensitive humor. We must all try to get along. I promise to adopt a flexible attitude and be supportive as plans may need to be changed.
• Act as a servant-disciple of the local pastor or mission organization. I will respect the advice I am given concerning attire, eating and drinking, and other such traditions which will help me to assimilate into the local community. The host/church has given their very best of accommodations; I will be courteous.

In consideration of my participation on this mission trip, I acknowledge and agree that:

Preparedness

I am prepared physically, emotionally, mentally, and spiritually for this trip. The scheduling, environment, and other foreign country and travel conditions are not adverse to me. I will be flexible and have a servant attitude.

Behavior

I agree to conduct myself responsibly and respectfully, following the guidance of trip leaders at all times. I will abide by curfews, rooming arrangements, and any venue or festival rules.

Substance Use

I understand that the use of alcohol, tobacco, or illegal substances is strictly prohibited during this trip. Torchgrab Africa abides by U.S. law both in the United States and abroad; therefore, anyone under the age of 21 is strictly prohibited from consuming alcohol. Violation of these rules may result in dismissal from the trip at my own expense.

Alcohol consumption of any kind is not permitted during the trip, to avoid being a stumbling block to others. Reckless or poor judgment will not be tolerated and will result in dismissal from the trip at my own expense.

Leadership Authority

I grant to any of the Torchgrab Africa Missions leaders the right to represent me in decisions relating to my welfare or the group welfare during the trip. I will follow the suggestions made on my behalf.

Minor International Travel Authorization

Any minor traveling internationally without a parent or legal guardian must have completed and notarized travel authorization and medical power of attorney documentation before departure. Torchgrab Africa will not permit international participation without these documents under any circumstance. This policy reflects Torchgrab Africa’s firm commitment to child safety. Specific requirements may be coordinated on a case-by-case basis, and Torchgrab Africa will assist families in completing the necessary documentation.

Financial Responsibility

As a participant in this mission trip organized by Torchgrab Africa, I understand that I am financially responsible for paying the required trip fees by the indicated deadlines. I understand that failure to make timely payments may result in my inability to participate in the mission trip and that Torchgrab Africa is under no obligation to refund any payments made in the event that I am unable to participate due to my failure to make timely payments.

I further understand that any changes to the payment schedule must be approved in writing by Torchgrab Africa leadership. I acknowledge that this promise to pay is a binding agreement between myself and Torchgrab Africa and agree to be held accountable to the terms of this agreement.

Medical Fitness & Risks

I attest and certify that I am physically fit and have no medical conditions that would prevent me from performing my assigned duties which may include heavy lifting, high altitude, heat, etc... I am aware of the disease risks associated with foreign travel and I accept these risks.

Insurance

I understand that Torchgrab Africa does not provide travel or medical insurance. I am responsible for obtaining my own coverage.

 

Equipment Liability 

I am responsible for any personal instruments, music, or equipment I bring. Torchgrab Africa is not liable for loss, theft, or damage.

Liability Waiver

I waive any and all claims for damages against Torchgrab Africa, Torchgrab Africa leaders or their contracted agents, arising from death, injury, illness, disease, viruses, inconvenience, or in property damage or loss occurring as a result of this mission trip for any reason including but not limited to any negligent act or acts of Torchgrab Africa Missions, Torchgrab Africa Missions leaders or their contracted agents which may in any way cause death, injury, illness, disease, virus, inconvenience or property damage or loss to me.

Photo and Video Release 

I consent for photographs and/or video images to be taken of me by Torchgrab Africa, Inc. or a representative. I understand the images may be used for marketing purposes to share about Torchgrab Africa through website, print, digital, or social media. Photo consent and release by means of this form is REQUIRED to attend any mission trips. By consenting to photographs and/or video images, I understand I will not be compensated from any party. Although photographs and/or video images will be used without identifying information such as name, I understand it is possible someone may recognize me.

I hereby release Torchgrab Africa, Inc., its lead volunteers, and any third parties involved in the creation of or publication of marketing materials, from liability for any claims by me or any third party in connection with my participation. 

By signing below, I hereby certify that all personal information provided in this form is true, accurate, and complete to the best of my knowledge. I acknowledge and agree to abide by the terms outlined in the Covenant and Release Form, and I understand that the falsification, misrepresentation, or omission of any facts may result in denial of the opportunity to attend this trip.

*If an applicant is under the age of 18, a parent or legal guardian’s signature (acknowledging both the personal information provided and the terms outlined in the Covenant and Release forms) is required at the end of this document for acceptance.
