I understand that this booking request form does not guarantee me an appointment. I understand that I will be contacted for follow up via text message within 48 hours, and will be required to send a deposit via Venmo or Zelle within 24 hours of agreeing to an appointment time with the artist. I acknowledge that until the deposit has been received, my appointment has not been confirmed. I understand that this is a non refundable deposit. If I need to cancel or reschedule the appointment I will do this no later than 48 hours before my appointment to avoid losing my deposit. I understand that if I make changes to my design after submitting this request and booking the appointment the cost may vary from the quote I was given. I can reschedule my appointment only once without losing deposit. I have a 15 minutes grace period if showing up late on the appointment date. Once the grace period is passed without any notification to the artist, I will lost the deposit and need to reschedule my appointment.
