Photography, Visual Image Release

The Department of Theatre and Dance will be sending employees to rehearsals and performances to take visual/audio images for marketing and archival purposes. Please read the below paragraph from CSUS Image Release form and sign in the designated area. Thank you.
Please sign to CONSENT to the image release: I am at least 18 years of age and competent to sign this release. I have read this release before signing, I understand its contents, meaning and impact and I freely accept the terms.
If you DO NOT CONSENT, please recognize that all images and video taken at rehearsals and performances including your image will NOT be distributed or used for marketing purposes.

Tech:
11/23-11/25 5pm-10pm

Dress Rehearsals:
11/30-12/1 5pm-10pm

Alumni night performance:
12/2 5pm call, 7pm curtain

Public performances:
12/3-12/5 5pm call, 7pm curtain
12/6 12pm call, 2pm curtain

If Cast – Students will be enrolled in a 1-unit THEA 121 course.

Costume Fitting Expectations:
All performers cast should expect to be called in for an initial measurement appointment (approx. 15 min long) in the weeks following being cast. All performers will be called in for 1-3 costume fittings (30-60 min long) as the show approaches. All measurement and fitting appointments are scheduled during regular Costume Shop hours (8am-5pm).

I understand the time commitment involved in these productions. I am able to attend all of these important dates, I will be available during my stated availability above for costume fittings, rehearsals, media events, etc. I understand that no new conflicts will be allowed beyond what I've listed above.
