Application Form for the CWCB Measurement Device Installation Program

PLEASE READ, IMPORTANT:
- Please fill out a new form for each diversion measurement structure application.
- Please review the FAQs and eligibility requirements on the Diversion Measurement Website before you begin.
We recommend you gather the following materials beforehand to reduce the amount of time it will take you to fill out this form:
1. Coordinates - Provide the latitude and longitude of your proposed measurement device (if you don't have the latitude and longitude, you can use the map tool in this application).
2. Proof of Land Ownership - Confirm you own the land, otherwise you will need to provide written landowner acknowledgement, easement, or other documentation.
3. Diversion Measurement Structure Information -
  - Name of diversion measurement structure and what creek/river it diverts from
  - WDID (see instructions below on how to obtain this information) (optional)
  - Ditch dimensions (top width, bottom width, depth) (optional)
4. Photos - Four (4) or more photos of your diversion measurement structure and its access road:
  - Overview photo of the headgate or diversion.
  - Photo of the headgate or diversion looking downditch.
  - Photo of the measurement structure or proposed device location looking upditch towards the headgate or diversion.
  - Photo of an existing staff gauge and/or a side photo capturing the full width of the structure.
  - Photo(s) of access road if available.
5. Information about when your diversion measurement structure typically diverts and when would be a good time of year to install a measurement structure.
There is an option at the bottom of this form to SAVE & CONTINUE LATER (expand the last dropdown - "Irrigation Schedule") if you need to leave the form to gather photos, information, etc.

- If your measurement device is located on FEDERAL LAND and you do not have an existing authorization, reach out to your respective federal entity to inquire about the authorization status of your structure. You must have prior authorization to be eligible for this program.
  
  To inquire about status, send an email to your respective federal entity contact with the following information about your structure (at a minimum): “Hello, I am applying for the CWCB Measurement Device Installation Program. To be eligible for this program, I must receive prior authorization from your office for my structure.   WDID: Water District Identifier, WDID is a 7-digit unique identification number assigned by Division of Water Resources to each diversion measurement structure. - Explain the configuration of the spring diversion. How is water collected, diverted, and controlled, e.g. perforated pipe collection system, collection box, etc.? (required)

- In order to determine the appropriate meter size, please list the capacity of the pump, whether the pipeline is buried or not, the material and size of the pipeline and outlet.
