Brook Arts Center Theater Tech Class

Emergency Medical Authorization

In the event of an emergency, I authorize the Brook Arts Center, its staff, instructors, and designated representatives to seek appropriate medical attention for the student if I cannot be reached immediately.

I understand that every effort will be made to contact the parent/guardian or emergency contact before medical treatment is administered whenever circumstances allow. I accept responsibility for any medical expenses incurred as a result of emergency care.

Student Conduct Agreement

Students participating in the Tech Class are expected to:

* Follow all safety instructions and procedures provided by instructors and Brook Arts Center staff.
* Use all tools, computers, projectors, lighting equipment, sound equipment, cables, microphones, control boards, and other technical theater equipment responsibly and only as instructed.
* Operate tools and equipment only when authorized and supervised by the instructor or designated staff member.
* Handle all equipment carefully and return tools, equipment, cables, and materials to their proper locations after use.
* Follow proper procedures when setting up, operating, moving, or storing technical equipment.
* Never use equipment, tools, electrical components, ladders, catwalks, stage equipment, or other technical areas without permission and appropriate instruction.
* Keep work areas, backstage areas, technical booths, and equipment areas clean, organized, and free of hazards.
* Treat fellow students, instructors, volunteers, staff, and the Brook Arts Center facility and equipment with respect.
* Maintain a positive, cooperative, and safe learning environment.
* Immediately report damaged equipment, unsafe conditions, injuries, or other safety concerns to the instructor or staff member.
* Follow all instructions regarding appropriate behavior around electrical equipment, elevated areas, stage machinery, lighting equipment, and other potentially hazardous equipment.
* I understand that unsafe behavior, misuse or unauthorized use of tools or equipment, damage to Brook Arts Center property, failure to follow safety instructions, or refusal to comply with instructor or staff directions may result in removal from the program without refund.

Brook Arts Center Liability Waiver & Release

By registering for the Brook Arts Center Tech Classes, I acknowledge and agree to the following:

I understand that participation in tech classes may involve the use of stage areas, electronics, projectors, ladders, lighting, sound, tools, and other technical materials. While the Brook Arts Center and its instructors will take reasonable precautions to provide a safe learning environment, I understand that participation involves certain risks.

I voluntarily assume responsibility for any risks associated with participation in this program and agree to release, waive, and hold harmless the Brook Arts Center, its officers, directors, employees, volunteers, instructors, representatives, and agents from any and all claims, injuries, damages, losses, or liabilities arising from participation in the Tech Classes, except where caused by gross negligence or intentional misconduct.

I certify that the information provided on this registration form is accurate. I understand that students are expected to follow all safety instructions, respect the facility and equipment, and follow the direction of instructors and Brook Arts Center staff.

For students under 18, I confirm that I am the parent or legal guardian and have permission to authorize participation in this program.

I grant permission for the Brook Arts Center to photograph or record my child/student during classes and use these images or videos for promotional purposes, including the Brook Arts Center website, social media, newsletters, and marketing materials.
