VIDEO PERMISSION & RELEASE
Please read the following before submitting:
By signing below, I confirm that I am 18 years of age or older and that I created, own, or otherwise have the right to submit the video, audio, statements, and other content included in my submission. I voluntarily grant ConnectionPlus permission to use, reproduce, publish, display, distribute, and otherwise use my submitted video, audio, likeness, voice, and statements for ConnectionPlus communications, educational, promotional, advertising, and marketing purposes, including but not limited to social media, websites, email, presentations, digital advertising, and other print or digital materials. I understand and agree that ConnectionPlus may edit my submission for length, format, or presentation, including trimming or excerpting the video, combining it with other content, and adding captions, text, graphics, music, branding, or other production elements, provided such edits do not intentionally misrepresent my statements. I understand that I will receive complimentary access to one eligible ConnectionPlus self-paced training as consideration for submitting an eligible video, regardless of whether ConnectionPlus ultimately chooses to use or publish my submission, and that I am not entitled to additional payment, royalties, or compensation for ConnectionPlus's use of the content. I confirm that no minor appears in my submitted video and that my submission does not contain confidential or identifying information about a child or family, including names, case information, schools, agencies, diagnoses, locations, or other private information. I understand that ConnectionPlus is not obligated to use or publish my submission and that, once content has been published or shared publicly, ConnectionPlus may not be able to completely remove copies that have been shared, reposted, downloaded, or otherwise distributed by third parties. By signing below, I acknowledge that I have read, understand, and agree to this Video Permission & Release.
