1. VOLUNTARY PARTICIPATION / PARTICIPACIÓN VOLUNTARIA
EN: I confirm that I am at least 21 years old and voluntarily participate in donation collection, sorting, packing, loading, unloading, transportation support, cleanup, or related relief activities. I understand that I may refuse any task I consider unsafe.
ES: Confirmo que tengo al menos 21 años y participo voluntariamente en actividades de recolección, clasificación, empaque, carga, descarga, apoyo de transporte, limpieza u otras actividades relacionadas. Entiendo que puedo rechazar cualquier tarea que considere insegura.
2. ASSUMPTION OF RISK / ASUNCIÓN DE RIESGO
EN: I understand these activities may involve lifting, carrying, repetitive movement, use of carts or equipment, vehicle traffic, slips, trips, falls, cuts, strains, contact with donated goods, and other known or unexpected risks. I voluntarily assume these risks to the fullest extent permitted by law.
ES: Entiendo que estas actividades pueden implicar levantar y cargar objetos, movimientos repetitivos, uso de carros o equipos, tránsito de vehículos, resbalones, tropiezos, caídas, cortaduras, lesiones musculares, contacto con bienes donados y otros riesgos conocidos o imprevistos. Asumo voluntariamente estos riesgos hasta el máximo permitido por la ley.
3. RELEASE AND HOLD HARMLESS / LIBERACIÓN DE RESPONSABILIDAD
EN: To the fullest extent permitted by New Jersey law, I release and hold harmless the organizers, participating businesses or organizations, property owners, coordinators, volunteers, employees, agents, and sponsors from claims arising from my participation, except to the extent a claim cannot legally be waived, including claims based on gross negligence, reckless conduct, or intentional misconduct where prohibited by law.
ES: Hasta el máximo permitido por la ley de New Jersey, libero y mantengo indemnes a los organizadores, negocios u organizaciones participantes, propietarios, coordinadores, voluntarios, empleados, agentes y patrocinadores frente a reclamaciones derivadas de mi participación, excepto cuando legalmente no sea posible renunciar a ellas, incluidas las basadas en negligencia grave, conducta temeraria o conducta intencional cuando la ley prohíba dicha renuncia.
4. SAFETY & MEDICAL AUTHORIZATION / SEGURIDAD Y AUTORIZACIÓN MÉDICA
EN: I agree to follow safety instructions, use reasonable care, report hazards or injuries promptly, and not participate while impaired. If I am unable to consent during an emergency, I authorize reasonable emergency medical assistance. I understand I am responsible for my own medical expenses unless otherwise required by law.
ES: Acepto seguir las instrucciones de seguridad, actuar con cuidado razonable, reportar peligros o lesiones de inmediato y no participar bajo efectos que afecten mi capacidad. Si no puedo consentir durante una emergencia, autorizo asistencia médica de emergencia razonable. Entiendo que soy responsable de mis gastos médicos salvo que la ley disponga lo contrario.
5. PHOTO/VIDEO CONSENT / CONSENTIMIENTO DE FOTO/VIDEO
EN: I permit photographs or video taken during the relief activities to be used for non-commercial documentation, community outreach, and acknowledgment of the relief effort, without compensation. You may opt out in the question below.
ES: Autorizo el uso, sin compensación, de fotografías o videos tomados durante las actividades para documentación no comercial, difusión comunitaria y reconocimiento del esfuerzo de ayuda. Puede no autorizarlo en la pregunta de abajo.
6. ACKNOWLEDGMENT / DECLARACIÓN
EN: I have read this waiver, understand it, have had the opportunity to ask questions, and sign it voluntarily. If there is any conflict between the English and Spanish text, both are intended to communicate the same terms; applicable law controls.
