Scholarship Requirement Acknowledgment

Senior students and parents, please read the following information, and sign to acknowledge that you are aware of the requirements to qualify for the HEAT scholarship awarded at the end of the year.
I, Student's First Name* Student's Last Name* acknowledge that I must be enrolled into the HEAT ministry by November 22nd in order to qualify for the HEAT Scholarship. I also acknowledge that I must attend at least 5 HEAT meetings to qualify. I acknowledge that the scholarship will be open from January 3rd, 2027 to March 28th, 2026. If I do not complete the scholarship application before the closing date, I acknowledge that I will not be apply at a later date. I acknowledge that my high school transcript(s), community service hours, and youth ministry involvement will be used to determine how much money I receive based on a unbiased point system. If awarded a scholarship, I acknowledge that I will forfeit my scholarship if I do not submit the necessary paperwork before the date given on the Award Letter that will be sent to my email.
