I am interested in participating in the Energy Efficiency Program offered by Public Utility District No. 2 of Pacific County (District). By signing below, I certify that:

• The primary source of heat in my home/business is electric with no oil or propane heat present; and
• The agreement for work to be performed is solely between me and the contractor(s) I have selected to perform the work; and
• The District is not in any way responsible for the warranty or work performed by contractors in connection with energy efficiency measures; and
• Pre-approval by the District's Energy Efficiency Specialist is required prior to the purchase and/or installation of an energy efficient measure for that measure to be eligible.

I understand that if I wish to proceed with this project and if it is approved by the District, a specific amount of incentive funds will be set aside.

Current heating system in the home:

PUD No. 2 of Pacific County reserves the right to revise program rules and guidelines, including discontinuing all or part of the available incentives, at any time without prior notice. All projects, once approved, will be subject to the guidelines in place at the time of approval. Rebates cannot exceed the cost of the approved measure.
