Minor Media Release Form

Risen Church may capture photographs, video, and audio during children's and youth ministry activities, worship services, programs, special events, performances, classes, and other church-related activities. Media may also include photographs of artwork, crafts, projects, performances, or other original work created or voluntarily submitted by a child.

With parent/legal guardian permission, Risen Church may use such media for legitimate church and ministry purposes, including church communications, ministry promotion, educational materials, Risen Church websites, social media accounts, livestreams, printed publications, presentations, promotional materials, and other Risen Church-controlled platforms and communications.

Risen Church will seek to use children's media in a respectful and appropriate manner. Risen Church will not intentionally publish a child's full name together with an identifiable photograph or video of the child without additional authorization from the parent/legal guardian.

I understand that Risen Church services and events may be photographed, video recorded, or livestreamed and that my child may appear incidentally in the background of general congregation, crowd, or event footage. Risen Church will make reasonable efforts not to intentionally photograph or feature a child whose parent/legal guardian has declined media consent.

CHANGING OR WITHDRAWING CONSENT

This media consent will remain in effect unless an updated Media Consent & Release Form is submitted.

A parent/legal guardian may change or withdraw permission for future use at any time by submitting an updated form to Risen Church. Withdrawal of consent applies to future use and may not require Risen Church to recall, remove, or destroy media that was previously published, printed, distributed, or otherwise used in accordance with permission previously granted.
