• Houston Walk to Remember Memory Sign Order Form

  • MEMORY SIGN INFORMATION:

    Personalized Memory Signs are available for $50 with your baby's name and DOB/DOD.  Memory Sign orders will be accepted through September 30th. All signs ordered will be displayed along the pathway for the Walk. The signs are yours to keep to take home at the conclusion of the Walk. 

    It is not necessary to order a new sign each year.  M.E.N.D. will gladly reserve a place for your sign for subsequent annual walks for an additional $25 donation each year. If you have a sign from a previous year, please see the FAQ for information on how to have your sign included in this year's Walk.

    For each baby to be remembered on your personalized memory sign, please provide a full name and date of birth/death on the order form. You can add up to 6 names to one memory sign. The size of the font used on the sign will be adjusted based on the number of names.
