* This field is required.This Credit Repair Service Agreement (“Agreement”) is entered into between Scale Wealth Financial Services (“Company”) and the individual identified above (“Client”). CLIENT RESPONSIBILITIES The Client agrees to: Provide truthful, accurate, and complete information. Provide requested identification and proof-of-address documentation when necessary. Provide access to or copies of applicable credit reports. Forward relevant credit bureau, creditor, collection agency, or furnisher correspondence to Scale Wealth Financial Services. Notify the Company of material changes to the Client’s contact information. Review documents submitted on the Client’s behalf when requested. Avoid knowingly providing false information for use in a dispute. The Client remains responsible for making payments owed to creditors and maintaining existing financial obligations. Hiring Scale Wealth Financial Services does not suspend or eliminate the Client’s obligations to creditors.

* This field is required.CREDIT MONITORING AND CREDIT REPORT ACCESS The Client may be required to maintain access to current credit reports or a three-bureau credit monitoring service so that Scale Wealth Financial Services can review changes and dispute results. Any third-party credit monitoring subscription is separate from the fees charged by Scale Wealth Financial Services unless expressly stated otherwise. The Client is responsible for maintaining any required third-party account during the service period.PRIVACY AND CONFIDENTIALITY Scale Wealth Financial Services may receive sensitive information from the Client, including identification documents, credit reports, account information, and other personal information. The Company will use such information for legitimate purposes related to providing the contracted services and will take reasonable measures to protect Client information. Client information will not knowingly be sold to unrelated third parties for marketing purposes without authorization.

Scale Wealth Financial Services does not provide debt settlement, lending, legal representation, or guarantees of credit approval.
