Unity in the Community 5K Registration

Each registrant MUST complete a Release Waiver in order to participate
Release of Waiver and Liability Agreement
Assumption of Risk: I acknowledge that participating in a 5K run/walk involves risks, including but not limited to falls, contact with other participants, the effects of weather, traffic, and conditions of the course. I voluntarily assume all such risks, both known and unknown, even if arising from the negligence of the event organizers or others.

Release and Waiver: In consideration of being permitted to participate in this event, I, for myself, my heirs, executors, administrators, and assigns, hereby release, waive, discharge, and covenant not to sue the event organizers, sponsors, volunteers, landowners, municipalities, and their employees or agents (the "Released Parties") from any and all liability, claims, or demands arising out of my participation in this event, including injury, illness, disability, or death, whether caused by negligence or otherwise.

Medical Treatment: I consent to receive medical treatment that may be deemed advisable in the event of injury, accident, or illness during this event, and I release the Released Parties from any claims resulting from such treatment.

Fitness to Participate: I attest that I am physically fit and sufficiently trained to participate in this event, and that I have not been advised otherwise by a qualified medical professional.

Media Release (optional): I grant permission to the event organizers to use photographs, videos, or other media of me taken during the event for promotional purposes, without compensation.

Severability: If any portion of this agreement is found invalid, the remaining provisions shall remain in full force and effect.
