Registration fee includes all access to all scheduled events.  

Enter the names and contact information for the people attending; if additional space is required, please list them and their contact information in the "Special Needs and/or Comments" box at the end of the form prior to submission.

Lodging: There is no on-site lodging available.  However, MMHC has reserved two cabins within the park complex, Ivy and Trillium.

 

They are located along the Huron River, just minutes from the River Hawk Conference Center. Each cabin has a large common room, microwave, refrigerator and air conditioner.  There are 2 private rooms in each cabin (A and B) with a bunk bed in each room.  Room A has a twin top bunk over a double bottom bunk bed.  Room B has a twin top bunk over a twin bottom bunk bed.  There are restrooms and showers located within 300 feet of the cabins.

• Bring your own linens, blankets, pillows, etc.
• Restrooms/showers are located within 300 feet of the cabins.
• Broom clean and remove all trash before leaving on Sunday.

Waiver of Liability:  The MMHC requires all people read and sign a Waiver of Liability before participating in MMHC events.

WAIVER OF LIABILITY

I hereby acknowledge and accept that there are inherent risks involved in the collection, identification and ingestion of wild mushrooms. I realize that mushroom forays are held in public woodlands where natural hazards do occur, immediate medical attention may not be available and foray leader may not be trained in emergency treatment. I further understand that people can and have known or unknown food allergies and people can experience gastric disturbances from ingesting wild mushrooms.

In consideration of this acknowledgement, and my voluntary participation in activities relating to the Michigan Mushroom Hunters Club (MMHC), having read this waiver and understanding the risks involved in participating in MMHC events, and of the agreement by the MMHC to allow me to participate in its activities.

I hereby release, on behalf of myself, my successors, heirs, assigns, executors and administrators, the MMHC, its officers, directors, members and volunteers from any claims of liability or demand whatsoever, including, but not limited to, bodily injury, sickness, disease, death, property loss or damage, or any other loss or damage of any kind which may arise out of or in connection to my participation in MMHC events, whether resulting from negligence or from some other cause.

I have read and understood the forgoing Waiver of Liability and, by signing below and clicking submit, I indicate my agreement. It is my intent to be legally restrained from asserting any claim connected herewith and I understand this agreement is unconditional and may not be waived by any person for any reason whatsoever.
