    Sign-Up and Permission Form

  • Students planning to audition for the Mid-State Choirs can attend this field trip, where students from across the district will come together to learn the audition music for the MTVA Mid-State Choirs. 

    Students can either:

    • Ride an SCHS Activity Bus from SCHS to LifePoint and back (first 59 students get a spot); or
    • Provide your own transportation to LifePoint (go directly to LifePoint at 8:45am) and back to SCHS (back to SCHS by 2pm).

     This field trip is only for students who plan to audition for the Mid-State Choirs.

     

    SCHEDULE

    8:20am           If you are riding the Activity Bus, come to Choir Room

    8:45am           Everyone arrives LifePoint, Smyrna
                            506 Legacy Dr, Smyrna, TN 37167

    9:00am           Warm-Up/Rehearsals

    11:20am         Lunch (bring sack lunch)

    12:00pm         Rehearsals

    1:30pm           Depart LifePoint

    2:00pm           Arrive SCHS – Everyone goes to 4th Block

  • I give my child, {studentName}, permission to travel to LifePoint Church in Smyrna on September 4, 2026 with the Stewarts Creek High School Choirs.  

    • I understand that my child must bring a sack lunch, bottled water, music, and pencil to this field trip.
    • I understand that my child must make up all work missed from this excused absence from class.
    • I understand that my child must follow all RCS policies and procedures while on this trip.
    • By signing up for this field trip, we are committing that my child will audition for Mid-State on September 25-26 (see Mid-State Gig Sheet/Sign-up on the Forms/Handouts portion of www.StewartsCreekChoirs.com)
