This student agreement is made between the student and the Berne Union Local Schools Work-Based Learning (WBL) Program. By signing this document, the student agrees to participate responsibly in the WBL Program and comply with all expectations.

• Maintain satisfactory academic performance as defined by Berne Union Local Schools.
• Demonstrate professional behavior, including punctuality, responsibility, and respect.
• Comply with all Berne Union policies as outlined in the student handbook, workplace policies, safety rules, and supervisor instructions.
• Complete all required assignments, reflections, and evaluations related to the WBL Program.
• Attend scheduled check-ins with the Work-Based Learning Coordinator as needed.

• Report any changes in work schedule, job duties, or workplace conditions to direct supervisor and WBL Coordinator.
• Report any safety concerns, injuries, or incidents at the workplace.
• Submit required time logs, forms, and evaluations by assigned deadlines.

• The student understands participation in the WBL Program is a privilege and may be revoked for failure to meet program expectations.
• The student understands that WBL hours count toward graduation requirements as outlined by the school.
• The student agrees to uphold the standards of Berne Union Local Schools and the partnering employer.
• The student acknowledges that communication and professionalism are essential components of this program.

By signing below, both parties acknowledge that they have read, understood, and agree to abide by all terms and expectations outlined in this Student Agreement.
