MEDIA CONSENT

EVENT TITLE: Trunk or Treat or Trick or Treat

            I consent and grant permission to the DALLAS ALUMNAE CHAPTER of DELTA DELTA DELTA (“DAC”) and the THETA KAPPA CHAPTER of DELTA DELTA DELTA (“THETA KAPPA”), its members and employees and its outside media representatives to photograph, video record, audio record and interview me and my child/ward.

            I further consent and grant permission to DAC and/or THETA KAPPA to copyright, use, and publish the photographs/video recordings/audio recordings, along with the name, age, city of residence and personal story of me and my child/ward, to increase public awareness of DAC and/or THETA KAPPA and their programs.   I also consent and agree that, for the purposes of increasing public awareness of DAC and/or THETA KAPPA and their programs and for other related purposes, such photographs,/video recordings/audio recordings, along with the name, age, city of residence and personal story of me and my child/ward may be (1) published in newspapers, magazines, journals and other printed materials; (2) distributed to the press; (3) broadcasted on radio, television and social media; (4) shared with outside entities/individuals associated with DAC and/or THETA KAPPA programs and (5) included in videos, audio recordings, websites, social media and presentations.

            I understand that I have the right to revoke this Consent at any time provided that it is in writing, except to the extent that ether DAC and/or THETA KAPPA has taken action in reliance thereon. To revoke this Media Consent, I understand that I should send my revocation in writing to DAC via email at dallasdeltas.president@gmail.com and to THETA KAPPA in writing to: Theta Kappa Chapter of Delta Delta Delta, attn: Chapter President, 3100 University Blvd, Dallas, TX 75205.
