2027 Individual Reconditioning Order Form

THANK YOU FOR CHOSING MERCURY SPORTS TO RECONDITION YOUR PERSONAL FOOTBALL HELMET.

Standard Individual helmet reconditioning turn around time is 4-6 weeks. Orders can be process faster - see our "RUSH CHARGE" below.
Please completely fill out this form. Be accurate and specific. If you have questions please reach out to us prior to submitting form.
Helmets that are dated 2017 or older are automatically rejected so do not send in for reconditioning.
Pricing on this form are base prices. Any parts that are missing or damaged will be replaced and that cost will be added to the base price.
All painted helmets MUST be repainted. Any helmet painted with non-certified paint, such as spray paint, cannot be reconditioned.
When your helmet is complete, we will call for payment information. Payments can be made debit/credit card (4% fee) or Venmo. Payment must be received before your helmet will ship back to you.

Before you submit this form, you will have the opportunity to review, edit, and print prior to submitting. Print 2 copies of the order form. Put your helmet in a box and include one copy of this order form in the box. Ship UPS, USPS, or FED Ex to the following address:

Brand new, never used helmet. Must still have original tags from manufacturer Does not have to be reconditioned, but still has to be completely broke down to paint.
