  • Authorization and Aknowledgement:

    To the best of my knowledge, my child is in good health and has not been exposed to any infectious disease in the past four weeks.

    I understand that participation in MYO Archery Camp involves outdoor recreational activities, including archery, and that these activities involve inherent risks. These may include, but are not limited to, injury from archery equipment, slips and falls, uneven terrain, weather conditions, physical activity, and the actions of other participants.

    I understand that MYO will take reasonable precautions to provide appropriate instruction, supervision, equipment, and safety procedures; however, not all risks associated with participation can be eliminated.

    I confirm that the participant will follow all safety instructions, range commands, camp rules, and directions provided by MYO staff and instructors.

    I voluntarily consent to the participant taking part in the program and acknowledge and accept the inherent risks associated with these activities.

    I desire my child to participate in the full camp program and all the activities (including and not limited to use of bows and arrows, camp fires, etc), unless I advise you otherwise in writing above.

    I have read the Rules and Regulations and agree with them and I have made my child aware of them as well. I recognize that all the people organizing and staffing the camp are volunteers. I will not hold anybody involved with the camp, or any other person liable for any reason, from any act which may arise at the Camp.
