WHEN TO USE THIS FORM: Use this form for renewal of existing courses from this provider. This form also allows for possible changes to teacher educators.

MEMBERSHIP FEE: If this is an institution such as a university, an annual course endorsement membership fee of $70 is required. This fee covers all course endoresment applications for the entire year. Use this link to make a payment.

TEACHER EDUCATORS

Fill in the information below about each Teacher Educator (TE). Add a row for each TE.

NON-ENDORSED INSTRUCTORS: ALL TEs MUST obtain endorsement through the "Teacher Educator Registry Application". This application to be completed by:

• New TEs
• TEs teaching a different Basic Orff level
• TEs teaching a new strand (recorder, vocal, movement). (TEs teaching recorder, vocal or movement do not need a new application to switch levels.)
