Liability Release
I understand that participation in activities at x99Lives Center, including but not limited to video gaming, esports activities, tournaments, recreational activities, group games, use of gaming equipment, and other event activities, involves certain inherent risks. These risks may include, but are not limited to, slips, falls, collisions with other participants, equipment-related injuries, and other unforeseen injuries or accidents.
I voluntarily allow my child to participate in the Parents' Night Out event and assume the ordinary risks associated with participation. To the fullest extent permitted by applicable law, I agree to release and hold harmless 99Lives LLC, its owners, employees, contractors, volunteers, and representatives from claims, demands, causes of action, damages, or liabilities arising out of my child's participation in the event, except to the extent caused by gross negligence, reckless conduct, or intentional misconduct where such liability cannot legally be waived.
I understand that 99Lives LLC staff will make reasonable efforts to provide a safe environment but cannot eliminate all risks associated with participation.
In the event of an emergency, if I cannot be reached, I authorize 99Lives LLC staff to seek emergency medical treatment for my child as deemed reasonably necessary. I understand that I am responsible for any medical expenses incurred.
I certify that the information provided in this registration is accurate and that I am the parent or legal guardian of the child listed above, or otherwise have legal authority to sign this agreement on the child's behalf.
