  • Coach/Team Officials: Will be added to your team roster.

    Before adding someone to this form:

    • They must have a valid Respect in Sport Activity Leader (RIS-AL) or The Shift Forward Coach (TSF-C) certification.
    • Don't submit a coach/team official who does not have valid RIS-AL or TSF-C

    If their RIS-AL is expired or they have never taken it:
    They must complete the TSF-C course before being submitted.

    How to check:
    Ask the coach/team official to log in to their Spordle Coach Profile → Qualifications. They can click on the course to see the expiry date. This is the information you need to provide on this form.

    Criminal Record & Vulnerable Sector Check (CRC/VSC)

    Requirement: Mandatory for all coaches/team officials being added to the roster.

    Action Required: They must upload their application receipt or completed check immediately to the Documents section of their Spordle Coach Profile.

    Verification: Officials can prove compliance by showing you the uploaded document directly in their Spordle profile under Documents.

    Have your name listed on and sign game sheets (which coaches should do)
    Be on the bench in an emergency

    Treasurers:  Will not be added to the roster.

    U11-U18 teams require 1 certified team official

    U7 and U9 teams require 2 certified team officials

    This four-hour online course covers injury recognition, fair play and ethics, rink safety, and Emergency Action Plan (EAP) procedures.

    The Safety Person will be equipped to support team health, injury response, and safe Return to Play practices.

    Certification is valid for three seasons and expires September 1 of the third season.

    Safety Person:  Must be one of your rostered coach/team officials

    Certification: Submitted coaches and team officials have a valid RIS-AL / TSF-C

    Roster Verification: Upon receiving the official roster, I will verify that all members are accounted for and that their first and last names and birthdates are correct. 

    Errors:  If any information is incorrect or missing, I will submit a Roster Correction form immediately. Members are not cleared to participate until I receive and verify the updated roster.

    Fees per team: 

    U11–U18 (Tiers A, B, C, AA, AAA): $72.91 per roster addition.

    U7, U9, & U11–U18 (Tier D): $72.91 above the allotted 5

    Practice coaches must be on a roster to be on the ice.

    Your Bench must mirror your roster, if they are not on your roster, they are not on the bench!!  

    There can be no more than 5 coach/team officials on my bench!!
