EMD Registration & Enrollment Form

ALL DANCERS MUST BE PICKED UP ON TIME.

Parents/guardians are responsible for making arrangements for their dancer to be picked up promptly at the scheduled end of classes, rehearsals, camps, and other EMD activities.

Eloquent Movement Dance LLC will not be responsible for dancers who remain at the studio after the scheduled end of an activity.

By submitting this form, I acknowledge that I have read, understand, and agree to the financial obligations, payment policies, participation requirements, and dancer pick-up policy established by Eloquent Movement Dance LLC.

ALL FEES ARE NON-REFUNDABLE.

Eloquent Movement Dance LLC accepts the following forms of payment:

Cash
Debit Card ( Zelle or Bank Transfer)
Credit Card ( same as Debit Card )

At this time ALL PAYMENTS ARE ACCEPTED IN THE STUDIO ONLY.

All payments must be submitted in the required amount by the designated due date.

TUITION & REGISTRATION

Creative Movement
$75 per month — 1 class per week
$105 per month — 2 classes per week

EMD Academy
$125 per month

EMD Foundations
$150 per month

Annual Registration Fee
$45 annually

Late Payment Policy
Tuition payments are due on the 1st of each month. The 1st through the 5th are the grace period. Payments received after the 5th will be considered late and will be subject to a $30 late fee beginning on the 6th.

Dancers with outstanding tuition or fees will not be permitted to participate in classes, rehearsals, or other EMD activities until all outstanding balances have been paid in full.

Concert Fee
The concert fee is required for dancers participating in the annual EMD concert. The payment deadline will be communicated to families by Eloquent Movement Dance LLC.

Sibling discounts do not apply to the concert fee.

Finale T-Shirt Fee
$25

All dancers participating in the annual concert are required to purchase the designated finale T-shirt. The payment deadline will be communicated to families by Eloquent Movement Dance LLC.

Ticket Sales
All dancers participating in the annual concert are required to sell a minimum of 10 tickets.

Ticket Price: $35 per ticket

Ticket money must be submitted by the deadline communicated by Eloquent Movement Dance LLC.

Annual Concert
The annual EMD concert date and additional event details will be announced once finalized.

I understand and accept the financial requirements established by Eloquent Movement Dance LLC.

By submitting this form, I acknowledge and agree to the following:

I am responsible for paying all tuition, registration fees, concert fees, ticket obligations, merchandise fees, and any other required fees by the established deadlines.

I understand that a dancer with an outstanding balance may not participate in classes, rehearsals, or other EMD activities until all outstanding fees have been paid in full.

I understand that all fees and payments are non-refundable.

I understand that Eloquent Movement Dance LLC accepts cash, debit card, and credit card payments in the studio only.

I understand that failure to satisfy all financial obligations may result in the dancer being excluded from the annual concert or other designated EMD performances and activities.

I understand that all dancers participating in the annual concert are required to meet the established ticket-selling requirement.
