PARTICIPATION WAIVER & RELEASE OF LIABILITY
I, the undersigned parent or legal guardian of the participant listed in this registration form, acknowledge and understand that participation with Amanda Marie Training and related movement programming involves physical activity and inherent risks, including but not limited to falls, collisions, sprains, fractures, illness, and other potential injuries associated with dance, tumbling, acrobatics, conditioning, flexibility training, and athletic movement activities. I certify that the participant is physically able to participate in these activities and agree to disclose any relevant medical conditions, injuries, allergies, or physical limitations that may affect participation. I understand that Amanda Marie Training will take reasonable precautions to promote a safe training environment; however, I voluntarily assume all risks associated with participation. In consideration for participation in this program, I hereby release, waive, and discharge Amanda Marie Training, Amanda Bernett, studio rental facilities, assistants, contractors, and affiliates from any and all liability, claims, demands, actions, or causes of action arising from injury, illness, loss, or damages related to participation in the program, except in cases of gross negligence or willful misconduct.
The following selection serves as my agreement/signature for Liability Waiver acknowledgement.
EMERGENCY MEDICAL AUTHORIZATION
In the event of illness or injury requiring emergency medical treatment, I authorize Amanda Marie Training and its representatives to obtain emergency medical care for my child if I cannot be reached immediately. I understand that I am financially responsible for any resulting medical expenses.

TUITION POLICY AGREEMENT & PAYMENT
Fall 10-Week Acro Lab Training Series | TUITION

Registration to hold a spot in the class is complete once this form has been completed and tuition has been received.

Full Series Registration (Full/Prorated Tuition & Reserved Position) | $350 (10 sessions), $315 (9 sessions), $280 (8 sessions), $245 (7 sessions)

• All classes consecutive from start date (Offer ends after Session 4, October 11th)
• $35/class
• Reserves a spot for the entire session
• Capped at 6 full registrants

NOTE: Registration must occur before class on October 4th (Session 3) to avoid the $15 registration fee. Registrations submitted after the October 4th class will incur the $15 registration fee.

If registered any time before the 4th session on October 11th, the $35/class prorated rate will be honored even if a student does not begin attending until the October 11th (Session 4) class.

After class on October 11th (Session 4), students who have not yet registered will only be eligible to register for the 6-Week Class Pass or continue with the $40 drop-in rate.

6-Week Class Pass (6-Week Tuition & Reserved Position) | $225

• Any 6 classes during the 10-week session
• $37.50/class
• Reserves a spot for the entire session
• Capped at 8 registrants (e.g. If there are 6 full registrants, up to 2 students can join as [6-Week Class Pass] registrants)
• Additional classes may be attended at the $40 drop-in rate as space permits

Drop-In | $40/class

• Flexible week-to-week attendance
• Based on availability

Drop-In classes will be offered depending on weekly availability at $40/class. Students who have not registered for the full 10-week Acro Skills Lab, or 6-Week Class Pass, must still have a full Registration & Liability Waiver on file.

Withdrawal & Refund Policy: Tuition may be refunded on a prorated basis if a student completely withdraws from the 10-week series. The refund will be based on the number of unused classes remaining in the series at the time of withdrawal. Classes that have already occurred, whether attended or missed, are not eligible for a refund.

Refunds will not be provided for individual class absences, scheduling conflicts, vacations, or missed classes. Refunds or partial refunds may also be considered at the discretion of Amanda Marie Training in cases of significant unforeseen circumstances.

If Amanda Marie Training cancels the program before it begins, all tuition paid will be refunded in full. If the program is discontinued after the series has begun, tuition will be refunded on a prorated basis based on the number of remaining classes.

If Amanda Marie Training must cancel an individual session, a make-up class or alternative instructional opportunity will be provided when possible.

Attendance is strongly encouraged due to the progressive nature of the series curriculum. No refunds or tuition credits are provided for individual missed classes. If another appropriate class is available during the session, a make-up may be offered when space allows. If no other class is available, students may be eligible for one make-up session offered as a one-time make-up workshop at the end of the series, subject to availability and instructor approval.
