PARTICIPATION WAIVER & RELEASE OF LIABILITY
I, the undersigned parent or legal guardian of the participant listed in this registration form, acknowledge and understand that participation with Amanda Marie Training and related movement programming involves physical activity and inherent risks, including but not limited to falls, collisions, sprains, fractures, illness, and other potential injuries associated with dance, tumbling, acrobatics, conditioning, flexibility training, and athletic movement activities. I certify that the participant is physically able to participate in these activities and agree to disclose any relevant medical conditions, injuries, allergies, or physical limitations that may affect participation. I understand that Amanda Marie Training will take reasonable precautions to promote a safe training environment; however, I voluntarily assume all risks associated with participation. In consideration for participation in this program, I hereby release, waive, and discharge Amanda Marie Training, Amanda Bernett, studio rental facilities, assistants, contractors, and affiliates from any and all liability, claims, demands, actions, or causes of action arising from injury, illness, loss, or damages related to participation in the program, except in cases of gross negligence or willful misconduct.
The following selection serves as my agreement/signature for Liability Waiver acknowledgement.
EMERGENCY MEDICAL AUTHORIZATION
In the event of illness or injury requiring emergency medical treatment, I authorize Amanda Marie Training and its representatives to obtain emergency medical care for my child if I cannot be reached immediately. I understand that I am financially responsible for any resulting medical expenses.
The following selection serves as my agreement/signature for Emergency Medical Authorization.
PHOTO & VIDEO PERMISSION (Permission may be changed at any time. Amanda Marie Training will always obtain additional permission before sharing any identifiable images on social media or marketing materials.)
TUITION POLICY AGREEMENT & PAYMENT
Fall 10-Week Acro Skills Lab

Tuition is $350 ($35/class) to register one student for the 10-week class series. Registration to hold a spot in the class is complete once this form has been completed and tuition has been received.

Drop-in classes will be offered depending on weekly availability at $40/class. Students who have not registered for the full 10-week Acro Skills Lab must still have a Registration & Liability Waiver on file.

Withdrawal & Refund Policy: Tuition may be refunded on a prorated basis if a student withdraws from the 10-week series. The refund will be based on the number of unused classes remaining in the series at the time of withdrawal. Classes that have already occurred, whether attended or missed, are not eligible for a refund. Refunds will not be provided for individual class absences, scheduling conflicts, vacations, or missed classes.

Refunds or partial refunds may also be considered at the discretion of Amanda Marie Training in cases of significant unforeseen circumstances.

If Amanda Marie Training cancels the program before it begins, all tuition paid will be refunded in full. If the program is discontinued after the series has begun, tuition will be refunded on a prorated basis based on the number of remaining classes. If Amanda Marie Training must cancel an individual session, a make-up class or alternative instructional opportunity will be provided when possible.

Attendance is strongly encouraged due to the progressive nature of the series curriculum.

Students may be eligible for one make-up session offered at the end of the series for a missed class, subject to availability and instructor approval.
