                    3. Certificate of Liability Insurance
                    4. Valid ServSafe Certificate (or equivalent)
                    5. Food Allergen Awareness Certificate
                    Your application is not complete until ALL required documents are submitted. Please combine the required documents into ONE PDF file when possible.

I understand that food and beverage vendors are required to provide all listed licenses, permits, insurance, and food-safety certificates before participation is approved. I understand that all fees are non-refundable once my participation has been confirmed. Cancellations made less than 48 hours before the event are subject to a $150 cancellation fee.
